This fee includes most current flat fee services that are performed during the visit, with the exception of acupuncture, Foot Leveler Orthotics, Radiology Services, Lab Services and any nutritional supplements and products. A separate $20 charge will be incurred for acupuncture, regardless of other services a patient may or may not receive on a given date. Patients receiving acupuncture and additional services covered by our flat fee plan within the same visit will be charged a total visit cost of $40.

All Medicare and Medicaid claims will continue to be filed. Medicare and Medicaid claims will only be submitted for one service provided at a $20 fee; this is the maximum the patient is responsible for regarding covered flat fee services if Medicare/Medicaid does not provide payment. Acupuncture is a non-covered service and requires a $20 payment, regardless of other services provided. Therefore, patients will be responsible for the $20 fee associated with acupuncture if those services are provided.
